  how to



 timing

  • make time in your support shift to make this call
  • if you have exceeded rostered time, then you need to request an extension of your shift with roster.me WHEN you do the report (shift extensions are the exception, it’s best practice to do incident reports asap and with participants, loved ones and others as much as possible)

 what to reported

  • the death of a person with disability
  • serious injury of a person with disability
  • abuse or neglect of a person with disability
  • unlawful sexual or physical contact with, or assault of, a person with disability
  • sexual misconduct, committed against, or in the presence of, a person with disability, including grooming of the person with disability for sexual activity
  • unauthorised use of restrictive practices in relation to a person with disability
  • if there is / was / or may be a risk to someone's health, welfare, or safety
  • if you can tell that ‘Something’s going to happen if someone doesn’t do something about it.’
  • near misses
  • if YOU are injured, or nearly injured
  • if there is a hazard or danger or risk in services to you or your colleagues
  • if there is a hazard or danger or risk in services to the person with a disability or their loved ones / nominee / guardian etc
  • if you have seen or heard anything of concern
  • if in doubt... contact shiny things and discuss your concerns

 language

  • if police are involved make sure you get a report number, the officer’s name, ID, police station, phone number or other information so we can use it to follow up matters if required
  • swear words are fine
  • be objective (state facts)
